Whether you are a new member or have been with us for many years, this page will guide you through everything that you need to know as a member of our medical group.
Find a doctor near you.
You can select or change your Primary Care Physician at any time.
- Contact our Customer Service team by calling (818) 357- 5000 or (866) 654-3471. TDD/TTY can call (800) 735-2929 (English), and (800) 855-3000 (Spanish). Note: if your request is made before the 15th day of a given month, your new PCP will be effective as of the 1st day of the following month.
- Once your new PCP is selected, a new card will be available from your health plan by mail or online. If you don’t receive it within 2-3 weeks, please contact your health plan directly.
Schedule and prepare for your appointment.
Whether this is your first visit or an annual checkup, your PCP will guide your care. Please remember to share all medications and over-the-counter supplements you are taking with your doctor and be prepared to discuss any concerns.

How prior authorization works and how to check status.
Your PCP might refer you to a specialist, such as a cardiologist, endocrinologist, or other physician who will care for your specific medical needs. However, some services will need to be pre-authorized before they can be performed.
Here’s how it works:
- The prior authorization is mailed directly to your address on record.
- Your specialist physician is responsible for any follow-up requests that you will need after your initial appointment.

Your doctor or specialist may request lab work, x-rays or other tests. An authorization may be needed.
Download the Quest app and create an account to schedule and see your results.
Most laboratory tests are done by our partner, Quest Diagnostics. They have locations throughout California, including one near you.
You will need to create an account to log into the Quest Lab Portal.
Once you create an account, you will be able to schedule, view, cancel, or reschedule your appointments. You will also be able to view your results in detail through this app.
Schedule an imaging, X-ray or radiology appointment.
Radnet is our largest radiology vendor serving more than 150 California locations. Visit their website to find an imagining center nearest you to schedule your appointment. Some locations conveniently offer weekend appointments.

Centrelake is our second largest radiology vendor offering weekday and weekend appointments. You can also view all of your medical imaging records once you sign up through their Patient Portal.

Urgent Care Finder.
Sometimes, the unexpected happens. Should this occur, and you need immediate medical care, an urgent care center may be a better option than an emergency room. An urgent care center may provide shorter wait times and fewer out-of-pocket costs than an emergency room.
